Your Initial Visit
Your initial consultation with our provider will generally include a medical history, examination, and allergy skin testing. Please remember to stop all antihistamines 4 days prior to your visit. Antihistamines will reduce the accuracy of skin tests. You may continue to take your other prescription medications, including inhalers.
New Patient Forms
For your convenience, please download and complete all New Patient Forms and bring them to your first visit. You may also fax or email them prior to your visit.
Payment & Insurance
We work with most insurance companies and plans. Some insurance companies may require a referral to see a specialist, and we will help expedite this process. For any questions on insurance, please contact our office. Please bring your insurance card to every visit.
